Job Summary

Under the guidance and supervision of the Senior Monitoring, Evaluation, Accountability and Learning Officer, the Health Information Assistant will manage records to ensure smooth information flow and access for decision making. The role involves using standard MoH/Donor data tools, compiling data, and submitting periodic health activity reports to maintain high standards of data collection and analysis.

Key Duties and Responsibilities

  • Facilitate information flow and records management for action and decision making.
  • Use and ensure the availability of standard MoH/Donor data tools and reporting forms.
  • Record, compile, use, and report data accurately.
  • Produce and submit periodic health activity reports.
  • Implement and support MEAL systems and approaches at the Facility.
  • Collaborate with MEAL and non-MEAL staff to ensure data quality and completeness.
  • Analyze facility data and work with program officers and SNR MEAL Officer for decision-making.
  • Update facility dashboards with key findings.
  • Ensure proper storage and availability of HMIS/HIS data tools.
  • Provide guidance on using HMIS/HIS data tools for consistency.
  • Requisition registers/tools and ensure their constant supply and proper use.
  • Participate in data cleaning and DQA activities.
  • Register all patients and summarize data according to reporting formats.
  • Provide data on notifiable/integrated disease surveillance and response.
  • Provide feedback to facility staff on health service provision.
  • Conduct CME sessions, mentorships, and review meetings on data management.
  • Participate in capacity building activities.
  • Adhere to Professional Code of Conduct and Ethics.
  • Perform other duties as assigned by the supervisor.

Accountability

  • Collaborate with SNR MEAL officer, MEAL Manager, and project team to document and follow up on client feedback.
  • Maintain an updated Client Feedback Registry.
  • Lead facility discussions on action plans from client feedback data review and analysis.

Learning

  • Document and share learning through performance reviews and data use meetings.
  • Analyze and present performance data monthly to inform decision making and learning at facility level.

Communication/Reporting

  • Provide regular feedback to the SNR MEAL Officer on activity implementation.
  • Prepare and submit daily, weekly, and monthly data reports.
  • Participate in the development and implementation of program work plans.

Minimum Qualifications

  • Education: Diploma in medical records and Health Management Information System from a recognized, licensed, and accredited institution.
  • Experience: At least one year of progressive experience in health record management. Minimum 2-year work experience in a busy health setting.
  • Skills: Applied knowledge in use of HMIS/HIS data tools, computer literate in MS Office applications, excellent communication and interpersonal skills.
  • Language: Good standard of spoken and written English; knowledge of languages spoken in the refugee settlement is an advantage.
