About US:
The Ministry of Health (MOH) has received a grant from the Center for Diseases Control and Prevention (CDC) Cooperative Agreement (CoAg), a strategic mechanism of the United States Government (USG) to implement activities in line with its mandate in support of the Public health response to HIV/AIDS and other health systems. CDC will be supporting a pool of Human Resources that are expected to lead the implementation of the grant. The staff to be recruited under this grant will be contracted by the MOH.
Job Summary: The Linkage and Referral Assistant will be conducting the intra-facility, inter-facility and bi-direction community-facility linkages and referrals component at the hospital and verifying that the clients/patients reached their referral points and their needs were met, or if not, the reason why their needs were not met and next step.
Key Duties and Responsibilities:
- Registering all referral cases into the referral register.
- Collecting all feedback receipt and completing the outcome column in the referral register.
- Ensuring that referring organization has received the feedback at the appropriate time.
- Tracking patients in case feedback was not received after 4-week period, through phone calls or emails and again recording the findings in the referral register
- Documenting and updating all community component related registers.
- Compiling monthly summary report and presenting to the Committee that deals with referrals.
- Establishing partnership with client that can be long-term, based on client’s needs.
- In charge of identifying client needs and referring them for services.
- Referring client for treatment and services, follow up with client and document referral activities.
- Advocating for client to meet needs across the continuum of care.
- Conducting quality assurance.
Qualifications, Skills and Experience:
- He / she must hold an “O” Level Certificate or its equivalent in Nursing, Public health or HIV/AIDS related courses.
- One (1) year working experience in a similar position
